We currently have an opportunity for an experienced Clinical Care Coordinator to join us at our Maylands WA Home.
About the Role:
- The role requires an in-depth understanding and experience in the Aged Care sector for delivering excellent clinical care for our residents.
- The role achieves this in accordance with the Aged Care Quality and Safety Commission standards. ACFI assessments and reviews form part of the role.
- As a senior leadership role at the Home, it requires people centered leadership behaviors to be demonstrated.
- The Clinical Care Coordinator works alongside the Home Manager in a supportive and collaborative way to ensure quality of care is received by all our residents.
Qualifications & experience:
- Minimum of 5 years Clinical Management Experience, competent knowledge of Microsoft Office and an ability to provide Rights to Work and a NDIS worker screen check
- Experienced and passionate Aged Care Registered Nurse
- Efficient in Leecare is desirable and comfortable to use and learn new programs
- Knowledge and understanding of ACFI
- Ability to deliver training and education to the Clinical team
- Ability to provide expert clinical assessments and care planning within the Aged Care environment
- Extensive knowledge of the Aged Care Quality Standards and ability to address Audit requirements
- Supportive and collaborative leadership behaviors to develop employees
- Demonstrated ability to analyze clinical indicators and data
- A passion for continuous improvement to maintain a high standard of care for our residents
Tasks & responsibilities:
- Deliver and provide clinical care for all our residents in line with accreditation standards
- Document care plans and resident clinical information using our Leecare clinical management system
- Provide continuous improvement for our systems and processes
- Demonstrate people centered leadership behaviors to influence a positive attitude
- Educate, support and provide training to all our direct care employees
- Manage all ACFI processes, assessments and reviews including submissions
- Manage clinical procurement requirements within financial criteria
- Achieve and deliver on regulatory compliance
- Provide reporting on clinical indicators in line with benchmarking and audits
