About the role: We currently have an opportunity for an experienced Clinical Care Coordinator to join us at our Maylands Home. The CCC role is a key support to the Home Manager and as a senior leadership role at the Home, it requires an individual to be supportive and collaborative in their approach to ensure quality of care is received by all our residents.. Tasks & responsibilities - Deliver and provide clinical care for all our residents in line with accreditation standards
- Document care plans and resident clinical information using our clinical management system
- Provide continuous improvement for our systems and processes
- Educate, support and provide training to all our direct care employees
- Manage all assessments and reviews including admissions
- Manage clinical procurement requirements within financial criteria
- Achieve and deliver on regulatory compliance
- Provide reporting on clinical indicators in line with benchmarking and audits
What you will bring: - Experienced and passionate Aged Care Registered Nurse (minimum 5 years)
- Experience with resident assessments and accreditation requirements
- Exceptional interpersonal skills with the ability to build rapport at all levels
- Comfortable to use and learn new programs (Leecare knowledge highly desirable)
- Ability to deliver training and education to the Clinical team
- Intermediate knowledge of Microsoft Office
- Ability to provide expert clinical assessments and care planning within the Aged Care environment
- Sound knowledge of the Aged Care Quality Standards and accreditation requirements
- Supportive and collaborative leadership behaviours to develop employees
- Demonstrated ability to analyse clinical indicators and data
- A passion for continuous improvement to maintain a high standard of care for our residents
- Current AHPRA Nursing Regulation
- Current NDIS check clearance (or willing to obtain)
- Proof of Work rights in Australia
- Proof of COVID & Flu Vaccination
